Medical Authorization Form
Employers are encouraged to use this form to authorize medical care for a work-related visit, such as:
- Pre-Employment Physicals
- Post-Accident Drug Screens
- Work-Related Injuries

By completing the Medical Authorization form prior to your employees visit it ensures services are streamlined and prevents any delays in obtaining authorization upon their arrival.
